E-Sign consent & disclosure
By choosing to set up a payment plan, you agree to these terms. This means we will send you important documents and payment reminders by text message or email.
1. What You Are Agreeing To (Scope)
When you agree to these terms, you are saying yes to receiving all official notices, disclosures, and documents about this payment agreement (and related future transactions) electronically.
2. Your Right to Paper Copies
Paper Option: You always have the right to ask for any required document related to this agreement in paper form.
Withdrawing Consent: If you decide you no longer want electronic documents, you can withdraw your consent at any time (see Section 3).
Important: If you withdraw your consent, please know that this may cancel your payment plan.
3. Stopping E-Sign (Withdrawal) & Keeping Us Updated
How to Stop: If you want to stop receiving documents electronically, you must contact us using the official contact information provided by Levy & Associates.
Keep Info Current: You are responsible for telling us right away if your email address or phone number changes. This is necessary to make sure you keep receiving all communications.
4. What You Need to View the Documents
To open and save (retain) our electronic documents, you must have:
A device (computer, phone, or tablet) that can receive and save electronic files.
An up-to-date internet browser (like Chrome, Safari, or Firefox).
A PDF reader (software) to view files sent in PDF format.
5. Your Electronic Confirmation
By clicking "I Agree" or providing your consent electronically, you are confirming two things:
You have the necessary equipment and software listed in Section 4.
You can successfully access and view the electronic documents we provide.
We will keep a record of your consent for compliance purposes.
6. Future Changes
If our requirements for viewing or saving documents change (for example, if we start using a different file format), we will inform you. If those changes affect your ability to access the files, we will ask for your consent again. You still have the right to withdraw your consent at any time.
